The
focus of the traditional medical model of care
has been on curing illness, even when a person's
condition is known to be terminal. In the past,
little attention was paid to relief of suffering
and compassionate care of the dying. Hospice
programs arose throughout the country in response
to a need for alternative models.

"Together with VNSNY Hospice
Care, we have developed a program of hope and
humanity, staffed by people who combine sensitivity
and compassion with expert knowledge and skill,"
says Judy Ingrassia, RN, Vice President
of Silvercrest's Nursing Services, and the liaison
with VNSNY Hospice Care. "This program
is built on comfort and support and is designed
to honor people's wishes and respect family
values and cultural beliefs."
When an eligible resident has been identified,
Ms. Ingrassia and the Silvercrest/VNSNY Hospice
Care team meet with the family to create an
individual, customized care plan for that resident's
needs. Services can include one-on-one home-health
care and a range of psychosocial support to
the resident and family.
Goals
- Comfort: Nurses work with doctors to evaluate and manage
any pain and other symptoms.
- Choice: Peoples wishes, values and cultural beliefs are respected
- Support: Chaplains and social workers provide emotional support while
dietitians and physical therapists contribute their expertise.
"A hospice is not a place, but an approach
to care," Ms. Ingrassia explains. "Bringing
this approach into Silvercrest makes complete
our mission of providing the full continuum
of compassionate care."
