Residents of Silvercrest present an
enormous challenge because of their frailty and multiple chronic conditions.
In the care of these and superimposed acute problems, a comprehensive
and detail-oriented approach is essential. |
Since each clinical decision can have repercussions upon
a chain of medical, social, psychological and spiritual issues, our team
is devoted to an interdisciplinary approach to each resident. All ancillary
functions are believed crucial to proper outcomes for our residents, prompting
us to include the opinions of physicians, nurses, therapists, dietitians,
social workers and recreational therapists in the treatment plan design.
While for some, full return to independence is a reasonable goal, it
is recognized that goals for others must be more modest. However, these
goals must always include the maximization of comfort, function and quality
of life. Key to achieving such ends is the willingness to regularly seek
out the patient's perspective and the wishes and expectations of the family. |

| The medical staff at Silvercrest is made up of a superb group
of full and part-time physicians, supported by an active staff of primary
care doctors and an extensive group of specialists from a variety of disciplines.
Many see patients on the floors while others are able to take advantage
of our unique, "Wellness Center."
This office-like environment has helped us to attract and keep superb specialists
who know that their time at Silvercrest
will be efficiently arranged and spent treating patients in a well structured
and organized environment. The Center and its specialists make the necessity
for outside referral less common, leading to greater continuity of care,
lower costs and less inconvenience to patients and families. |

No top-flight medical program can be sustained without an educational
backbone. We at Silvercrest are committed
to education in geriatrics and special extended care and believe that
we can only hope to attract tomorrow's best medical talent to extended
care medicine by exposing today's trainees to superb clinicians within
just such an environment.
At the current time, Silvercrest hosts
3rd year medical residents (from NYHQ) for their 1 month geriatric rotations
and also welcomes a Cornell Geriatrics fellow to a 1 month preceptorship
in Administrative Medicine. These rotators have the opportunity to discover
the challenging and gratifying career that can be found in "extended
care" medicine. Also, it is hoped that the facilities and resources
brought together through partnership of Silvercrest,
New York Hospital Queens and the New York Presbyterian Healthcare System,
will soon lead to the establishment of joint geriatric medicine and geriatric
dental fellowships. This will certainly position Silvercrest at the cutting
edge of geriatric and special care. |
